Some of these answers are WRONG! These are Bridge Size (2.25" x 3.5"). We are one of the sellers of this product here on Amazon (DK Outlet). The UPC code is 073854300039, the model number is 1007276 and these are Bridge Size cards. We have asked Amazon to update the listing.

No. They used to include this reorder card, but no longer.

Replacement card included in both decks?

I don't recall every seeing a replacement card and I've used these for several years. Excellent product.
