Does this need to re-pair every time it turns on?

No, both devices will re_connected automaticly after turn on if both devices has paired before. Otherwise, it need to pair or repair if any device to pair with others before works with MIC2 again.

I am wondering if multiple hearing aid users can pair with this to hear the speaker using it. Can Bluetooth earbuds pair with it.

I am unsure. I found it limited in it's capacity. I have not used it since. Now, having said this, I purchased this to replace a much superior product, which would allow me to pair it to my aids and use them like wireless ear buds. This product does not do this. My suggestion is that you present this question to the manufacturer for a more comprehensive response/explanation. Further, it is an adequate product. Just not the calibre of my former Bluetooth hearing aid device.
